13INside Track visits ASC and sees how integrated care helps residents

The program reduced the number of unnecessary ER visits and hospitalizations from residents at American Senior Communities.

Integrated Care Network at American Senior Communities collaborates coordinated care for individuals to make sure they are getting the right care at the right place and the right time. The goal is to provide residents with excellent quality care.

"We saw an opportunity to enhance the quality outcomes of those who discharged into the community and back into their homes," said Troy Reiff ,Vice President of Post Acute Service Development at American Senior Communities. "We narrowed our network of providers, homecare companies and hospice companies. Now we work together with them so we know what the outcomes are and can assure our patients and their families that when they go home they get the best quality care with our partners in that transition."

Reiff says not only has the program been well received by the patients and family members but they are seeing a tremendous reduction in unnecessary ER visits and hospitalizations that were occurring before the program was put into place.
