13 Investigates Pharmacy Records

A - Dumpster(s) very secure. Locked and/or unaccessible to public.
B - More than one dumpster. Some locked and secure. Some unlocked and accessible to public.
C - Dumpster(s) somewhat secure. Located behind closed fence but fence not locked. Public access possible.
D - Dumpster(s) not secure. Fence around dumpster(s) open or has no gate. Easy public access.
F - Dumpster(s) very unsecure. No fence. Dumpster lid left open. Very easy public access.
G - No opportunity to take trash.
H - Took trash and found customer information mixed in with garbage.
J - Took trash and found NO customer information mixed in with garbage.
K - Did not take trash because dumpster was empty at time of visit.
L - Did not take trash because dumpster was behind closed fence.
M - Found discarded pharmacy prescription label(s) with patient information.
N - Found pharmacy medication instruction sheets with patient information.
P - Found original prescription forms with patient information.
Q - Found pill/medicine bottle(s) or box(es) with patient information crossed out or partially ripped off but legible.
R - Found pill/medicine bottle(s) or box(es) with patient information NOT crossed out or ripped off.
S - Found customer computer order information with patient information.
(CVS recently purchased Osco Drug stores. Many Osco locations will soon become CVS pharmacies while others will be closed.)
