H - Took trash and found customer information mixed in with garbage.
J - Took trash and found NO customer information mixed in with garbage.
K - Did not take trash because dumpster was empty at time of visit.
L - Did not take trash because dumpster was behind closed fence.
M - Found discarded pharmacy prescription label(s) with patient information.
N - Found pharmacy medication instruction sheets with patient information.
P - Found original prescription forms with patient information.
Q - Found pill/medicine bottle(s) or box(es) with patient information crossed out or partially ripped off but legible.
R - Found pill/medicine bottle(s) or box(es) with patient information NOT crossed out or ripped off.
S - Found customer computer order information with patient information.
